TUMESCENT LIPOSCULPTURE

Tumescent liposculpture is performed by placing tumescent fluid into the fatty layer underneath the skin, this allows for easier sculpting of the tissues. The tumescent fluid is introduced into the fatty layer through various small incisions that are made in areas of natural skin folds to minimise scarring. The fluid contains normal saline, lignocaine and adrenaline. Lignocaine is a local anaesthetic, used in a very dilute form to numb the fatty area of the skin. Adrenaline constricts the blood vessels in the fat and skin, this makes the lignocaine work better and reduces the amount of bruising patients may have after the liposculpture procedure.

Excess fat is removed using a small hollow blunt-ended instrument called a “cannula”. The incisions made are quite small, do not need to be closed with sutures, and will usually close within days and fade within a few weeks.

What is liposculpture?

Liposculpture is a procedure that can help sculpt the body by removing unwanted fat from specific areas, including the abdomen, hips, buttocks, thighs, knees, upper arms, chin, cheeks and neck. During the past decade, liposculpture has benefited from several new refinements. Today, a number of new techniques, including the tumescent technique and laser-assisted liposculpture are helping many surgeons provide selected patients with more precise results and quicker recovery times. Although no type of liposculpture is a substitute for dieting and exercise, liposculpture can remove stubborn areas of fat that don’t respond to traditional weight-loss methods.

Who are the best candidates for liposculpture?

There is no definite age or weight limits. Most patients have localized fat deposits and some of our happiest patients are within ten to 14 kilograms of their ideal weight, and have been stable at that weight for years. Some patients have certain bulges irrespective of their weight and others are content with their weight, but would be more comfortable if certain bulges were re-contoured. The best candidates for liposuction are in good health and have realistic expectations of the benefits of liposculpture.

What do I expect at the initial consult?

In your initial consultation, we will evaluate your health, determine where your fat deposits lie and assess the condition of your skin. We will explain the body-contouring methods that may be most appropriate for you. Be frank in discussing your expectations with us. We should be equally frank with you, describing the procedure in detail and explaining its risks and limitations.

Would all my questions be answered?

Individuals considering liposculpture often feel a bit overwhelmed by the number of options and techniques being promoted today. However, we can help. In deciding the right treatment approach for you, we will consider constructiveness, risks, cost and appropriateness for your needs. We do this using our clinical judgement, a skill that is developed through thorough training and experience.

What shall I do prior to my procedure?

You will be given specific instructions on how to prepare for your procedure, including guidelines on eating and drinking, smoking, and taking or avoiding vitamins, and certain medications. If you develop a cold or an infection of any kind, especially a skin infection, your procedure may have to be postponed.

At the time of the consultation, you will be given a request to have basic pathology tests done prior to your procedure. Because you will have sedation for your surgery, you cannot drive and you need to arrange for someone to drive you home after the procedure or we can organise a taxi for you.

Where will my procedure be performed?

Your procedure will be performed in our specially equipped surgical facility using sedation and local anesthetic. The Tumescent Technique means liposculpture can be performed totally by local anesthetic. Single areas, i.e., neck, abdomen, knees, outer thighs, etc., and multiple areas can be treated, in a given session, depending on the number and size of the areas.

What type of anesthesia am I going to have?

Together, we will select the type of anesthesia that provides the best level of comfort for your surgery. If only a small amount of fat and a limited number of body sites are involved, liposculpture can be performed under local anesthesia, which numbs only the affected areas. However, the local is usually used along with sedation to keep you more relaxed during the procedure.

What happens on the day of my procedure?

On the day of the procedure, you will answer a pre-procedure questionnaire and after discussing the areas to be treated again, these areas will be marked. The time required to perform liposculpture may vary considerably, depending on the size of the area, the amount of fat being removed, the type of anesthesia and the technique used. After the procedure, you will stay in recovery for an hour after which you can leave with your nominated driver.

How does the technique vary?

The basic technique of liposculpture, as described above, is used in all patients undergoing this procedure. However, as the procedure has been developed and refined, several variations have been introduced.

Tumescent liposculpture, a technique in which a medicated solution is injected into fatty areas before the fat is removed, is commonly used today. The fluid – a mixture of salt solution, lignocaine (a local anaesthetic) and adrenaline (a drug that contracts blood vessels) — helps the fat be removed more easily, reduces blood loss and provides anesthesia during and after surgery. Fluid injection also helps to reduce the amount of bruising after surgery.

ProLipo (Laser-Assisted Liposculpture) is a minimally invasive laser-assisted lipolysis procedure for ablating and melting unwanted fat. The technique involves the use of fibre-delivered laser energy to the target fatty tissue resulting in the destruction of the fat cells. The laser also produces skin tightening through its collagen stimulation and remodelling. The procedure is ideally used in combination with liposculpture, but it can be used as a stand-alone procedure. ProLipo requires the use of tumescent local anesthesia with or without sedation. There is minimal downtime and recovery takes only a couple of days. In addition to ablating and melting fat, ProLipo also tightens skin through its collagen remodelling and stimulation effects. These results usually occur slowly over weeks to as much as nine months.

What happens after the procedure?

After the procedure, you will experience some fluid drainage from the incisions. To control swelling and to help your skin retract, you will be fitted with a compression garment to wear over the treated area for a period of two weeks. You will also be prescribed antibiotics to prevent infection.

Don’t expect to look or feel great right after surgery. Even though the newer techniques are believed to reduce post-operative discomfort, you may still experience some pain, burning, swelling, bleeding and temporary numbness. Pain can be controlled with simple analgesia, though you may still feel stiff and sore for a few days.

It is normal to feel a bit anxious or depressed in the days or weeks following surgery. However, this feeling will subside as you begin to look and feel better.

When do I return to normal activity?

Healing is a gradual process. You should start walking around as soon as possible to reduce swelling and to help prevent blood clots from forming in your legs. You will begin to feel better after about a week or two and you should be back at work within a few days following your procedure.

Strenuous activity should be avoided for about a month as your body continues to heal. Although most of the bruising and swelling usually disappear within three weeks, some swelling may remain for three months or more.

What about my new shape?

You will see a noticeable difference in the shape of your body quite soon after surgery. However, improvement will become even more apparent after about four to six weeks, when most of the swelling has subsided. After about three months, any persistent mild swelling usually disappears and the final contour will be visible. You will find that you are more comfortable in a wide variety of clothes and more at ease with your body. And, by eating a healthy diet and getting regular exercise, you can help to maintain your new shape.

Is Liposculpture a treatment for obesity?

Liposculpture is not a treatment for obesity and is not an alternative to dieting. The procedure works best on those whose weight is stable and/or has localized areas of fat deposits.

What areas of the body can benefit from this technique?

The most frequently treated areas in women include the waistline, abdomen, outer thighs, inner thighs, buttocks, knees, ankles, and neckline. In men, the most commonly treated areas are the love handles, breasts, abdomen, and neckline.

How are the benefits of Liposculpture measured?

The cosmetic success of liposculpture relies on removing centimetres rather than kilograms. Little weight is actually removed and yet dramatic change in silhouette is possible. Most important is the patient’s satisfaction and increased self-acceptance.

The Tumescent Technique is the most significant advance in Liposculpture. It affects liposculpture totally by local anesthesia and thus avoids general anesthesia. The technique is considered the lowest-risk form of liposculpture as blood loss is greatly reduced, post-operative recovery time is reduced, and the cosmetic results are optimized.

What does Tumescent mean?

The word “tumescent” means swollen or firm. This technique is accomplished by injecting a large volume of a dilute solution of lignocaine, a local anesthetic and of adrenaline, a vasoconstrictive drug which shrinks capillaries. Patients find local anesthesia works so well that they have reduced pain postoperatively and in addition, no longer need general anesthesia.

Are there any scars as a result of the procedure?

The Tumescent Technique uses very small cannulas, which result in smaller incisions. Incisions are generally not sutured closed and heal with less scarring than those sutured. When possible they are concealed in natural body creases; such as the end of the buttock fold, the umbilicus, under the chin, or in the pubic hair. The small incisions close within a few days and fade with time.

How much bruising will I get afterwards?

Bruising is greatly reduced because the local anesthetic solution contains adrenaline which constricts blood vessels, reducing bleeding, and therefore bruising. The drainage of anesthetic solution following the Tumescent Technique further reduces bruising as it contains blood cells that would otherwise remain under the skin and appear as a bruise. By minimizing bruising and swelling, the tumescent liposuction accelerates the rate of healing.

Can fat re-accumulate in the contoured areas?

Fat cells are permanently removed as fat cells are thought not to multiply after puberty. As long as the patient does not gain excessive weight, the new silhouette is permanent. Fat removal, however, does not allow for abuse of diet later. Patients, after liposculpture, have sometimes found that areas treated by liposculpture retain the new contour whereas areas not treated have been new sites for fat development. As one gains weight the size of the individual fat cells will increase and too much weight gain may mask the benefit. The benefit will show when the extra weight is reduced. People with prior weight problems still have to watch their weight and exercise.

What complications may occur?

Complications of liposculpture are quite rare. As with any surgical procedure, liposculpture is associated with common side effects such as bruising, swelling, and numbness. Skin irregularities have been greatly reduced because Dr. Nasser uses very small cannulas that effect a more even and less traumatic reduction.
